Application for Change of Particulars in Chinese Medicine Pharmacy Licence and Registration Form


How to Apply

Time of application: Application shall be submitted to the Health Bureau for any change of particulars in the Chinese medicine pharmacy licence and registration form, including the pharmacy’s name, telephone number and/or fax number as well as the pharmacy owner’s identification document.

Application procedures and documents required:

  • Application documents required for name change of a Chinese medicine pharmacy:
  1. Completed FC-11 Application Form for Change of Particulars in ‘Chinese Medicine Pharmacy’ Licence and Registration Form;
  2. Signboard design of the Chinese medicine pharmacy (Note 1);
  3. Photocopy of updated Business Tax Declaration Form (M/1 format) obtained from the Financial Services Bureau (Note 2).
  • Application documents required for change of telephone number and/or fax number of a Chinese medicine pharmacy:
  1. Completed FC-11 Application Form for Change of Particulars in ‘Chinese Medicine Pharmacy’ Licence and Registration Form;
  • Application documents required for change of identification document of the Chinese medicine pharmacy owner (Notes 3-4):
  1. Completed FC-11 Application Form for Change of Particulars in ‘Chinese Medicine Pharmacy’ Licence and Registration Form;
  2. Certified true copy of updated identification document, or present the original of the aforesaid document (for verification and certification by the Pharmaceutical Affairs Department of the Health Bureau).

Notes:

  1. The applicant shall alter the pharmacy signboard according to the approved signboard design after the application for name change of Chinese medicine pharmacy is approved, and shall submit relevant photo(s) to the Pharmaceutical Affairs Department of the Health Bureau;
  2. Can be submitted after the application for name change of Chinese medicine pharmacy is approved;
  3. Refer to the identification document of an executive management member if the pharmacy owner is a legal person;
  4. If the owner also holds the licence(s) of other business(es) engaging in pharmaceutical activities, please provide a list of the business(es) concerned.

Service hours:
Mondays to Thursdays: 09:00-13:00; 14:30-17:45
Fridays: 09:00-13:00; 14:30-17:30
Closed on Saturdays, Sundays and public holidays


Fees

Not required.


Application Processing Time

The Health Bureau will finish the review and approval process within 90 days upon receipt of all the application documents required.
