Application for Deadline Extension of Chinese Medicine Pharmacy Establishment

How to Apply

Time of application: Application must be submitted to the Health Bureau before the deadline of premise establishment.

Application procedures and documents required:

  1. The completed FC-10 Application Form for Deadline Extension of ‘Chinese Medicine Pharmacy’ Establishment;
  2. Documents justifying the deadline extension (if applicable).

Location of application: Pharmaceutical Affairs Department, Health Bureau
Address: Avenida Sidónio Pais, n.º 51, Edif. “China Plaza”, 1.º andar, Macau (Receipt and Dispatch Unit)
Service hours:
Mondays to Thursdays: 09:00-13:00; 14:30-17:45
Fridays: 09:00-13:00; 14:30-17:30
Closed on Saturdays, Sundays and public holidays

Application Processing Time

The Health Bureau will finish the review and approval process within 90 days upon receipt of all the application documents required.

Remarks/Notes for Application

  1. According to Paragraph 2 of Article 6 of Decree Law no. 53/94/M dated 14th November, it is necessary to obtain the approval of the facility inspection committee indicated in Paragraph 6 of Article 9 of Decree-Law no. 58/90/M dated 19th September in order to establish a premise;
  2. According to Paragraph 4 of Article 19 of Decree Law no. 58/90/M dated 19th September, the applicant/legal person has six months to establish a premise, and may apply for an extension of the establishment deadline if they can prove that the delay in establishment is not caused by them on purpose.
