Application procedures and documents required

  1. A duly completed Request Form of Certificate of Medical Sanitation;
  2. A photocopy of the applicant’s identity document, and the original or certified true copy to be presented for verification;
    [Not required for applications made in the name of a company / organization that have submitted the document, as long as the identity document remains valid];
  3. A photocopy of the death certificate of the deceased, and the original or certified true copy to be presented for verification;
  4. A photocopy of the deceased’s identity document;
  5. Receipt of exhumation service issued by The Municipal Affairs Bureau.

Health Bureau – Centre for Disease Prevention and Control

Service hours: Mondays to Fridays: 09:00~12:30, 14:30~17:00
(Services provided on an appointment basis on Saturdays, Sundays and public holidays)
