Application procedures and documents required
- A duly completed Request Form of Certificate of Medical Sanitation;
- A photocopy of the applicant’s identity document, and the original or certified true copy to be presented for verification;
[Not required for applications made in the name of a company / organization that have submitted the document, as long as the identity document remains valid]; - A photocopy of the death certificate of the deceased, and the original or certified true copy to be presented for verification;
- A photocopy of the deceased’s identity document;
- Receipt of exhumation service issued by The Municipal Affairs Bureau.

Certificate of Medical Sanitation for Human Body Transportation to be issued on the next working day. (Performance Pledge)
