Application procedures and documents required
For application:
- Original and a copy of the certificate issued by a Health Bureau doctor;
- Copy of the applicant’s Macao SAR Resident Identity Card (present the original);
- One recent photo (1-inch) of the applicant (if applying for a physical blue card).
Note: Upon staff’s receipt and verification of the relevant documents, the applicant will receive a certified copy of the certificate, whereby the applicant is temporarily entitled to free healthcare during the processing period.

Remarks/Notes for Application
To facilitate the renewal process, applicants should apply for the necessary medical certificate in advance:
- If you are consulting a doctor: apply to the doctor directly;
- If you are not consulting a doctor: apply two months before the expiry date of the Healthcare Card at the Medical Report Reception Counter (located on the G/F of Outpatient Building) (Please specify the purpose as “Apply for Healthcare Card for Special Patients”);
- Hematology-oncology patients: apply at the reception desk of the Hematology-oncology department one month before expiry and pick-up two weeks after application.
Application Status Enquiry and Result Collection
For collection:
Electronic Blue Card
After receiving the approval notification, you can bind and use it in the “My Card Wallet” of the One Account.
Physical Blue Card
If you have opted to apply for a physical card during the application, please bring the original ID card and the duplicate confirmation copy of the certificate to the registration counter in the lobby of the Centro Hospitalar Conde de São Januário to collect it after receiving the approval notification. If a third party is collecting it on your behalf, they must present the original ID cards of both parties.
