How to Apply

Application procedures and documents required

 

For first-time application, please submit:

 

Newborn

1.     Birth record certificate (commonly known as “birth certificate”);

2.     Original identification document of the mother;

3.     Clear Chinese or Portuguese address (utility bills such as water or electricity bills);

4.     Infant health handbook (blue/pink health booklet) (if available);

5.     Vaccination proof document (if available)

 

Other individuals

1. A duly completed application form;
2. Copy of identity document (present the original; Macao non-residents must also present a valid travel document or permanence authorization);
3. Legible proof of address, either in Chinese or in Portuguese (e.g. water bill, electricity bill, etc.);
4. Proof of medical benefits (if available).

For replacement / access to information, please submit:

1. A duly completed application form;
2. Copy of identity document (present the original; Macao non-residents must also present a valid travel document or permanence authorization).

For correction or update of information, please submit:

1. A duly completed application form;
2. Copy of identity document (present the original);
3. Proof of updated information (e.g. certificate issued by the Identification Bureau).

1. Lobby of Conde de São Januário General Hospital — Registration Office: Monday to Friday , except government holidays, 08:30~20:00
2. Health centres – registration counter (please check contact list of health centers)

Application Processing Time

The “gold card” can be registered and applied at the lobby of Conde de São Januário General Hospital within the following time frames:

-During rush hours (08:30~18:00), within 30 minutes. (Performance Pledge)

-During non-rush hours (18:00~20:00) , within 20 minutes. (Performance Pledge)

Remarks/Notes for Application

1. If applied by a third party, a letter of authorization established in front of a public or private notary / proof of parental relationship / proof of legal guardianship will be required;
2. For update or merging of information, the old Patient Card should be returned to the Health Bureau upon application;
