How to Apply?

Application Period:Applications are accepted throughout the year.

Application Procedures and Documents Required:

Complete the application form; the form can be obtained at the Education and Youth Affairs Bureau or downloaded from the DSEJ website.

The declaration can be obtained within 3 working days of receipt of the completed application form and required documents. (Service commitment)


Enquiry on the Process of the Application and Obtaining the Service Results

Obtain the Service Results:

Collect in person: the applicant needs to produce his/her original identification document.

Collect by an authorised person: the authorised person must submit a copy of his/her identification document, a copy of the applicant’s identification document as well as a written authorisation of the applicant.
