How to Apply
Submission Period：
Applications are accepted at any time
Application Procedures and Documents Required：
Applicant may choose to:
(1) Submit the application form online and then visit the main entrance service counter in person at the UM Sports Complex to settle the payment and collect the user card.

(2) Visit the main entrance service counter in person at the UM Sports Complex to fill in the application and then settle the payment. UM Sports Facilities User Card will be ready to be picked up immediately:
(2.1) If the applicant is an UM alumnus, a copy of the alumni card (or the electronic version in the “Macao One Account”) and a photo are required for the application
(2.2) If the applicant is the direct family member of an UM employee, a copy of the immediate family member’s staff ID card and a photo are required for the application
Application Location and Office Hours
In-person application：
Visit the main entrance service counter in person at the UM Sports Complex
Address: Ground floor, UM Sports Complex (N8), Avenida da Universidade Taipa, Macau, China
Service counter opening hours: 07:00-23:00 every day (except the first to third day of the Chinese New Year)
Online application：
Applicants may submit applications online and then visit the main entrance service counter in person at the UM Sports Complex to settle the payment and collect the user cards.

Charges
Holders of Family Member Card: Free of charge
Others: MOP150 per year / MOP75 half a year

Remarks/Notes for Application
If you have submitted your “One Account” information during application, the user card will also be displayed in your “One Account” e-card package.

Documents need to be presented or submitted upon receipt of the service results：
Valid identification documents need to be presented
