Documents required to be submitted:
1. Application form(012/DICV/DSAGM);
2. Legible photocopy of Health Certificate of the animal(s);
3a. Legible photocopy of identity document(s); or
3b. Legible photocopy of Registration for Operation of Foreign Trade issued by the Economic and Technological Development Bureau;
4. Photocopy of valid license of the animal(s). (Note: Only applicable to animals subject to licensing, e.g. dogs and horses.)

1. Collection: 3 working days, dated from the application date;
2. Application for declaration and for the import license could be submitted at the same time;
3. The declaration is valid for 30 days, dated from the issuing date.
