  1. Declaration – Relationship with the deceased (011/DHA/DHAL);
  2. A clear photocopy of the identification document of the applicant;

Application fee:

  1. 6-month storage at most for the deceased above the age of 7 (renewal for once only): MOP2,000.00
  2. 6-month storage at most for the deceased at or under the age of 7 or stillbirth (renewal for once only): MOP1,000.00

Stamp duty: 10% of application fee
Security deposit: Not applicable

Remarks / Important notes on application

Important notes:

Statement of reasons for the application for renewal is required to be made by the original applicant. IAM may grant an exception for an extension of storage up to 6 months.
