Processing procedure and required documents
Documents required to be submitted:
- Application form for cremation service of bone remains (013/DHA/DHAL);
- A clear photocopy of the identification document of the applicant;
- Declaration of cremation of bone remains (Declaration)
Additional documents to be submitted (those that must be submitted when applicable):
- If the bone remains are from a public cemetery, the applicant is required to submit a photocopy of the receipt of remains exhumation;
- If the bone remains are not from a public cemetery, the applicant is required to submit a clear photocopy of the identification document of the deceased and the supporting document(s) of the source of the bone remains.

The original of the identification document of the applicant is to be produced.

Remarks / Important notes on application
Important notes:
- pplicants are required to deliver the bone remains of the deceased to Our Lady Mercy Cemetery on their own within 10 working days counting from the day of application to schedule for cremation;
- In accordance with the stipulation of Article 13-B of the current Administrative Regulation no. 37/2003 “Regulations on Management, Operation and Supervision of Cemeteries”, the ashes should be collected within 90 days counting from the day of cremation of the bone remains, otherwise it is considered as abandonment of the ashes concerned, and IAM can handle the ashes properly without further notice.
