Application procedures and documents required: A written document containing the following information should be submitted together:

  • Particulars of the person delivering the products: e.g. name, contact information (if any);
  • Product information: name, quantity, lot number (if any), date of expiry (if any), etc.;
  • Reason for submission: expired, recalled or damaged medicine, etc.;
  • The person delivering the products must sign and stamp (if applicable) on the document.

Mondays to Thursdays: 09:00-13:00; 14:30-17:45

Fridays: 09:00-13:00; 14:30-17:30

Closed on Saturdays, Sundays and public holidays
