Documents to be submitted:

1. Application form for registration of intention of environmentally friendly burial(016/DHA/DHAL );

2. A clear photocopy of identification document of the registrant.

Remarks / Important notes on application

Important note: 

1. This registration is voluntary in nature for individuals who have planned their burial methods in advance to register. Those who intend to choose environmentally friendly burial for their own after-death arrangements can make a registration of intention to allow the family members and friends to know about the intention of the registrants after registration, so that they can assist in application for environmentally friendly burial (tree burial or garden burial) service in the future.

2. The registrants can provide the information of two contact persons at maximum, and they should obtain the consent from the contact persons in advance and inform the family members and friends of the intention concerned.

3. Making a registration indicates that the registrant gives the consent to IAM for notifying the contact persons of the intention concerned.

4. The burial methods selected by the registrants are their personal preference, and the actual burial methods will be selected by the family members and friends.

5. In case of amendment of information or cancellation of registration, the application is required to be made by the registrant to IAM.

6. After the registrant passes away, when the family members and friends process the death registration formality for the registrant, the Civil Affairs Registry will inform the family members and friends of the intention concerned, so as to facilitate the after-death arrangements to be made according to the intention of the registrant.

7. After completion of the registration procedure, the registrant’s intention and the information provided will be used for registration and notification purposes by IAM and the Civil Affairs Registry.

8. Environmentally friendly burial is to bury the ashes into the soil to allow the deceased to be at rest and return to nature. At present, the location of environmentally friendly burial is the Taipa Sa Kong Municipal Cemetery, with the options of tree burial and garden burial. The burial method will be subject to the methods available for use at the time.

9. After completion of the registration of intention of environmentally friendly burial, IAM will produce the registration card of the intention concerned and the applicant can choose the location for collection of the card. If the applicant chooses to register at the Service Office in Our Lady Mercy Cemetery, the registration card can be collected on the day in general. If other service locations are chosen, the registrant can proceed to the designated card collection location to collect the registration card after receiving the notification. Registration cards not collected for more than 3 months will be destroyed and no replacement will be issued.
