Remark/Application Instructions
1. The expression “Segunda Via” (second copy) is stated on the reissued registration card and it has the same valid period as the original registration card;
2. If the applicant is a non-resident worker, the copy of identification document submitted is required to be consistent with the information on the veterinary professional accreditation certificate. Moreover, a copy of the Non-resident Worker’s Identification Card (blue card) is required to be submitted additionally.

The Municipal Affairs Bureau issues the Registration Card of Veterinary Surgeon to the individuals concerned
Methods for Collection of Service Result
Collection in person:
The applicant is required to produce the original receipt and the original identification document for verification.
Authorising a representative for collection:
If the applicant intends to authorise another person to collect the service result on behalf of him / her, the representative is required to produce the original receipt and the original identification document for verification. In addition, an authorisation letter is required to be submitted along with a copy of the identification documents of the authoriser and the authorised person.
