Remarks/ Important notes on application
1. The following information should be stated clearly in the application letter:
1) The number of the import license;
2) The name of the license holder;
3a) The number of the identity document(s) of the license holder; or
3b) The number of Registration for Operation of Foreign Trade issued by the Economic and Technological Development Bureau to the institution applying for the license;
4) Reason(s) for the cancellation;
5) Signature of the license holder or authorized person;
6) Date of application.
2. For application made for imported dogs, applicant can submit at the same time an application for the refund of license fee for dogs. The guidelines are listed as below:
1) The relevant refund application should be made within the same year the applicant applies for the import license;
2) Applicant should file the relevant application through the application letter for cancellation of import license;
3) Applicant should submit the original receipt issued by IAM.
