Procedures and required documents
1. Mod. No. 3A (available at the service counters of the Transport Bureau or download from the Transport Bureau website).
2. Original of a valid Macao Special Administrative Region Resident Identity Card or a valid document for legal stay in Macao SAR (in case of evaluation and approval, the document is digitalized and saved for the records)
3. One 1’5 inches recent color photos (photograph specification) or choose the photo for identification documents under “My photos” in “Macao One Account”( List of contracted merchants on “ Macao One Account” ) of 12 months or less from the shooting date.
4. Declaration of lost and stolen of driving license (to be provided by service counters of the Transport Bureau).

Remark/Application Instructions
1. In accordance with the “Macao Traffic Regulation”, supplementary document must be submitted within 90 days upon the date of submission of the application, otherwise, the application has to be reapplied all over again and related payment must be paid.
2. Applicant who intends to change the personal particulars is required to show the original certificate of personal data issued by the Identification Bureau and submit a photocopy of it.
3. If the applicant’s driving license is about to expired during this procedure, the renewal of the driving license must be done simultaneously.

Note: If the collection location is at Macao Government Services Centre / Macao Government Services Centre in Islands, and that the document has not been collected for one year since the scheduled collection day, please visit the service centre of the Transport Bureau (Estrada de D. Maria II) for enquiry and arrange for the collection.
