Application Constraints

If applicant is over the age of 21, the Identification Services Bureau (DSI) is not allowed to issue his/ her Certificate of Special Record for Minors. (Article 29 of Decree-Law No. 27/96/M)

Application Procedures and Documents Required

A queue ticket is required in advance. For the methods to obtain the ticket, please refer to the website of the Identification Services Bureau (DSI). Also, the intended purpose of applying for the Certificate of Special Record for Minors (please refer to the list of purposes) must be provided when lodging the application.

(1) If the applicant is aged 12 or above and under the age of 16, the application must be submitted by his/her legal representative (parents or guardian), please submit the followings: 

• For applicants not holding a Macao SAR Resident Identity Card:
  – A photocopy of his/her valid travel document or the valid document used for entry into Macao;
  – Proof of the applicant’s connection with Macao;
  – A clear original set of his/her ten fingerprint impressions (Blank form for fingerprint – Fingerprint Form for Certificate of Criminal Record Application.

• Original identification document of the applicant’s representative;
• Proof of relationship (if the Identification Services Bureau does not have the relevant data);
• Proof of the applicant’s inability to apply in person (such as a medical record).

(2) If the applicant is 16 years old or above:

(a) Application lodged by the applicant

• Present the applicant’s Macao Special Administrative Region Resident Identity Card;
  
• For applicants not holding a Macao SAR Resident Identity Card:
  

• The applicant’s valid travel document or the valid document used for entry into Macao.
  
• Documentary proof of the connection between the applicant and Macao.
  

(b) Application lodged by an authorized person

Required documents:

• An applicant without a Macao SAR Resident Identity Card is required to submit a photocopy of his/her valid travel document or the valid document used for entry into Macao, along with documentary proof of the connection between the applicant and Macao.
  
• An applicant without a Macao SAR Resident Identity Card has to submit a clear original set of his/her ten fingerprint impressions. (Form for fingerprint – Fingerprint Form for Certificate of Criminal Record Application);
  
• Original identification document of the applicant’s representative;
  
• Documentary proof of the kinship (if the Identification Services Bureau does not have the relevant data);
  
• Documentary proof of why the applicant cannot apply in person, such as medical record.
  

(c) Application made by authorized person

• An original copy of declaration (If there are any cross-outs or amendments in the declaration, the declarant should sign beside the changes as confirmation. The signature should be identical to the signature on the declarant’s identification document). Download the declaration form (Authorization for Certificate of Criminal Record Application);
  
• For applicants not holding a Macao SAR Resident Identity Card:
  

• The applicant’s valid travel document or the valid document used for entry into Macao.
  
• Documentary proof of the connection between the applicant and Macao.
  
• A clear original set of the applicant’s ten fingerprint impressions. (Form for fingerprint – Boletim Dactiloscópico destinado ao pedido do certificado de registo criminal);
  

Original identification document of the authorized person.

* If the applicant’s identification document does not bear his/her signature (e.g., Hong Kong Identity Card or Permit for Travelling to and from Hong Kong and Macao), it is necessary to submit an original power of attorney bearing the applicant’s signature that is authenticated by a competent institution, or a photocopy of other identification document bearing the applicant’s signature (e.g., the signature page of travel document) for verification.

Fees and Payment Methods

Fees:

• Standard Service: MOP50.00
• Express Service: MOP150.00
• Applicants under the following circumstances who lack financial capability to pay the applicant fee may apply to this bureau for a fee exemption, which is subject to approval:

Types	Requirements
Unemployment	1) Applicants have to sign the application form for fee exemption; 2) Applicants have to sign a declaration to declare their unemployment status; 3) In order to prove the lack of financial capability due to long-term unemployment, applicants have to submit receipts of Employment Registration issued by the Labour Affairs Bureau (DSAL) in the past 12 months.
Poverty	1) Applicants have to sign the application form for fee exemption; 2) Submit a photocopy of a valid Beneficiary Card issued by the Social Welfare Bureau (IAS). (The original document is required for verification)
Disability	1) Applicants have to sign the application form for fee exemption; 2) Submit a photocopy of a valid Disability Assessment Registration Card issued by the Social Welfare Bureau (IAS). (The original document is required for verification)
Note:  If necessary, this bureau may request applicants to provide additional supporting documents to verify their financial incapability.

Headquarter (China Plaza), Macao Government Services Centre in Venceslau de Morais and Macao Government Services Centre in Islands: Cash, credit card, Union Pay “QuickPass”, debit card, Macau Pass and MPay are accepted for payment.

Approval time required

• Normal application: 5 working days (Performance Pledge)
• Urgent application: 2 working days (Performance Pledge)

Notes:

1. The abovementioned collection date is calculated from the day after the application and all required documents are received by DSI. DSI reserves the right to adjust the processing time under special circumstances.
2. If the applicant chooses to collect the documents in Hengqin, the documents will take additional 5 working days to be collected (i.e. estimated collection date plus 5 working days).

The Certificate of Special Record for Minors is valid for 90 days from the date of issue.

Target users: 

Applicants who lodged the application for identity document or certificate at service counters or self-service kiosks.

Document Collection Procedures:

1. Scan or input your receipt barcode. (If the receipt barcode cannot be detected, please ask our staff for assistance.)
2. Be aware of your name and the corresponding counter number shown on the screen.
3. Collect your document or certificate at the designated counter.

Collect in person:

Verification of applicant’s identity is required at the time of document collection.  For identity card renewal or the former travel document has not expired, the former identity document must be brought along for cancellation.

Collect by an authorized person:

To authorize a representative to collect the document, the applicant has to fill out the authorization form on the receipt and sign the receipt according to the Macao SAR Resident Identity Card signature. Details of the procedure can be found at DSI’s Questions and Answers.

The Government Smart Locker – “Smart Collect”

Identification-related certificates can be collected through the Smart Locker – “Smart Collect”. If the applicant chooses to collect his/her documents at the “Smart Collect” at the Government 24-hour Self-service Center, he/she should collect the documents within 3 days upon the receipt of the collection notice form “Macao One Account” or “Business and Associations Platform”, otherwise the documents will be sent back to the Identification Services Bureau’s Collection Counter on 1/F of China Plaza for collection during office hours. For details about the Government Smart Locker – “Smart Collect”, please refer to the website of the Government 24-hour Self-service Center.If applicants choose to collect the documents in Hengqin, the documents will take additional 5 working days to be collected.
