How to Apply
Application at counter (applicants are required to obtain a number tag in advance. For the methods to obtain the tag number, please refer to the website of the Identification Services Bureau.)
A. Application formalities and required documents:
a. First-time application for the name of association and foundation
Documents required:
(1) A completed application form for the “Certificate of Adoptable Name of Association and Foundation”;
(2) Photocopies of valid identification document of the authorized representative and the founders stated in the application form;
(3) Draft constitution*;
(4) Relevant proof that all founders are qualified to execute the purpose of the association or foundation (e.g. proof of professional qualifications, proof of employment, documentation of relevant events previously organized, etc. If declarations or resumes are submitted, corresponding supporting documents are required.)
(5) If the domicile of the legal person is established in a non-residential property, a written consent from the property owner or proof of lease is required.

b. Changing the name of an established association or foundation
Application should be made at DSI in person by the president or chairperson of the established association or foundation, or a representative authorized by the above persons with the following documents:
(1) A completed application form stamped with the seal of the association or foundation;
(2) Photocopies of valid identification document of the president or chairperson of the association or foundation, or the representative authorized by the above persons. If the application is made by an authorized representative, an authorization letter is required;
(3) Draft constitution with the changed name of the association or foundation(1).
(4) If the domicile of the legal person is established in a non-residential property, a written consent from the property owner or proof of lease are required.
Remarks:
1. Please refer to Law no. 2/99/M “Regulations on the Right of Association” of 9 August and Article 140 to 172 of the “Civil Code” of the Macao SAR.
2. If the new name is approved, in pursuant to the stipulations of Article 163 (3) of the Civil Code, which states that the resolution to amend the constitution is supported by the vote of at least three-fourth of attending members in the General Assembly, the association/foundation should hold the General Assembly timely and submit the minute to DSI for follow-up.
B. Application Methods:
Applicants can lodge the application at DSI service centres in person or by a delegate.
Remarks/ Notes for Application
The founder of an association or foundation shall first apply for the “Certificate of Adoptable Name of Association and Foundation” at the Identification Services Bureau (hereinafter referred to as DSI). Upon receipt of the certificate, the founder shall notarize the establishment of the association or foundation at the Public Notary Office. After the constitution is published in the Official Gazette, DSI will notify the association or foundation of the registration number by an official letter.
