How to Apply

Application Procedures and Documents Required

Application at counter (applicants are required to obtain a number tag in advance. For the methods to obtain the tag number, please refer to “Methods to Obtain Tag Number and Appointment” of the website of the Identification Services Bureau.)

1. Target Applicants:

Member of the leadership structure of the association or foundation

2. Application Formalities and Required Documents:

Application can be made at DSI in person or by an authorized representative with the following documents:

a. A completed application form (Pedido de Certificado de Associação e Fundação) (affixed with the stamp of the association or foundation)

b. A Photocopy of the applicant’s valid identity document;

c. A power of attorney is required if the applicant is not a member of the leadership structure.

Fees and Payment Methods

Fees:

MOP15.00, extra MOP5.00 for each additional page
(Note: Administrative Public Welfare Legal Person is exempt from the fees and charges)

Payment Methods:

Headquarter (China Plaza), Macao Government Services Centre and Macao Government Services Centre in Islands  : Cash, credit card, Union Pay “QuickPass”, debit card, Macau Pass and MPay are accepted for payment.


Processing Time

12 working days(Performance Pledge)
(Note: counting from the following working day upon the receipt of completed application form and all necessary documents.)
