Processing procedure and required documents
1. Application form for licensing service for food & beverage establishments (MOD.028/DHAL) which can be obtained free of charge from IAM or downloaded from the IAM website www.iam.gov.mo; (not required for online applications)
2. If the license or registration certificate holder is a legal person, a photocopy of the valid Business Registration Certificate is required to be submitted (exemption for companies registered at the Commerce and Movable Property Registry of Macao; not required for online applications).
Note: If the identification document of the applicant (including individual and legal person) has been updated, a clear photocopy is required to be submitted upon application.
Documents required to be produced: The original or notarised copy of the identification document which bears the signature of the applicant or the legitimate representative is required to be produced.

Application by in-person visit: Process and reply to application for cancellation by license holder of License for Food and Beverage Establishments within 15 working days.
Remark/Application Instructions
1. The person concerned can make the application in person or authorise a representative to make the application;
2. If the applicant is an individual, the original identification document is required to be produced for verification upon application made in person;
3. If the applicant is a legal person, the application form is required to be signed by the legitimate representative and the original identification document is required to be produced for verification upon application made in person. Moreover, a photocopy of the valid Business Registration Certificate (exemption for companies registered at the Commerce and Movable Property Registry of Macao) and a clear photocopy of the identification document of the legitimate representative of the company are required to be submitted;
4. If the establishment has a license for signboards and advertisements, application for cancellation can be made at the same time.
