How to Apply

Application Procedures and Documents Required

  1. Application Approach
    1. The interested person or a legal agent makes the application.
    2. The agent should submit the following:
      1. A certified letter of authorization;
      2. A copy of his/her valid identification document.
  2. Documents to be Presented
    1. Unless otherwise noted, the original should be exhibited for authentication if a copy is submitted;
    2. The copy should be authenticated if the original cannot be exhibited.
  3. Documents to be Submitted
    1. A completed Declaration / application form (DARP/DARP M-3);
      [Note: (1) The form is available at the subdivision concerned and the website of the Public Security Police Force; (2) The application should contain: A. Interested person’s personal information; B. Fact to be certified; C. Purpose of certificate/declaration.]
    2. Copy of interested person’s valid identification document;
    3. Other documents as required by the Subdivision.
